Science Safety Contract

To the Students and Parents:
Safety is a primary concern in any laboratory situation. We want each student to be able to participate in "hands on" science investigations. The basic safety rule is to use common sense and good judgment at all times. Safety rules are discussed in the classroom, and we believe parents should also be informed. The following list, while incomplete, is mandatory:
1. Never use any equipment or materials except as directed by the teacher or the experiment directions. 2. Never throw anything. 3. Do not move any hot materials or equipment from one place in the room to another. 4. When near any open flame, very long hair must be tied back. 5. Eye protection devices must be worn whenever directed by the teacher or experiment instructions. 6. No running, bare feet, or horseplay allowed. 7. Never taste or smell any materials unless directed to do so by the teacher or experiment instructions. 8. Students are encouraged to help one another, but any interference with another student's work will be considered a safety violation. 9. Students must report immediately to the teacher if they notice any unsafe conditions, equipment, materials, or behavior. 10. Never start any exercise unless you have read and understand the instructions completely; if you have any question, ask the teacher.
Students who violate safety regulations will be given non-experimental assignments, a parent contact, office referral, or in repeated referrals or in gross misconduct, students will be dropped from the class with no credit. While these and other
rules of conduct in the science labs are discussed in class, we ask that you discuss them with your child, sign this paper, and have him or her return it to the science teacher as soon as possible. Your son or daughter will not be able to participate in lab until this contract is signed and returned.
